INSURANCE WAIVER I hereby release all rights and claims for damages against Precision Dance Association in the event of any injury resulting from dance class or any dance-related activities.

REFUND POLICY There will be absolutely no refunds after November 15th, 2025 except under exceptional circumstances and where approved by the Board of Directors. Before November 15th, 2025, a partial refund will be made for fees (based on full months remaining after time of withdrawal). There will be no refunds for partial months. To withdraw a dancer from class, email precisiondanceinfo@gmail.com with intent to withdraw. This must be done prior to the 1st of the month in order to receive a refund for that month. Verbal notification or non-attendance is under no circumstances considered automatic withdrawal.

PHOTO RELEASE WAIVER I hereby consent to have photographs or video of the above named dancer, taken by Precision Dance Association (PDA) or by a sub-contractor hired by PDA. This would include recital DVD’s, etc. I also consent to the use of these photographs or videos on our website or in other advertising /print material produced by PDA. I acknowledge that no payments or other considerations in any form will be due to myself, my child, heirs, or agents at any time due to use of such images.

I, as the legal guardian of the above named individual, hereby give my consent to the terms as they have been written. By clicking this box I am providing my consent via electronic signature.
